Appointments can be made by calling 813-879-1188 Monday through Thursday. For your convenience we can schedule as late as 5:00 pm. We can usually accommodate new patients within one to two weeks and our follow-up schedule is extremely flexible.
In order to see scheduled patients in the most timely manner, it is necessary for all patients to have a scheduled appointment. If you wish to be seen TODAY please call the office in the morning and we will try our best to accommodate you.
Sometimes running late cannot be avoided. However, when patients arrive late for appointments, it negatively impacts other scheduled patients. In order to help us maintain a timely schedule, we ask that you arrive on time. If you are more than 15 minutes late, we may have to reschedule your appointment. Please help us stay on time.
- Co-pays are due at time of check-in.
- Deductibles are due at time of check-in.
- Outstanding balances are due at time of check-in.
- Self-pays are due at time of check-in.
SPECIAL SELF-PAY FEES:
With the costs of health care plans rising, more and more people are uninsured or using MSAs. In an effort to serve our community, we have reduced our fees by 50% for uninsured or MSA patients who pay in full at the time of service. Providing quality care at an affordable cost is our way of supporting and ensuring the best of healthcare for each and every one of our patients.
Prescriptions will be given only during an office visit. Likewise, refills (other than those indicated on the original prescription) require an office visit. All narcotic prescriptions must be written and therefore cannot be called in to the pharmacy.
Your pharmacy may fax us a form for refill. Since we try to have appropriate refills on the original prescription, please make an appointment for a follow-up visit.
